How to Use Your Vision Insurance With Aldermiller
By Aldermiller Team
A step-by-step guide to claiming out-of-network reimbursement, with where to find each major insurer's claim information.
Aldermiller is not in network with any vision insurance company, and we can't bill your insurer or check your benefits at checkout. That does not always mean your plan won't help. Many vision plans reimburse part of the cost when you buy from a provider outside their network. You pay us at checkout, then send a claim to your insurer yourself.
This guide explains how that works, what to ask your insurer, what to send, and where to find each major company's claim information. Your insurer makes every decision about what it will pay, so we can't promise reimbursement.
|
The short version 1. Call your insurer and ask about out-of-network eyewear benefits (before you order, if you can). 2. Order from Aldermiller and pay in full. 3. Get your itemized receipt from us. 4. Fill in your insurer's out-of-network claim form and send it with the receipt. 5. Keep copies, and follow up if you haven't heard back. |
Step 1: Ask Your Insurer About Out-of-Network Benefits
Plans differ a lot. Some pay a fixed dollar amount toward out-of-network frames and lenses, some use a percentage, and some limit how often you can claim. Call the member services number on the back of your insurance card (or log in to your member account) and ask:
● Does my plan include out-of-network benefits for frames and lenses? What will it pay for each?
● Does it cover purchases from an online retailer, and does the seller need to be a licensed provider?
● How often can I claim (for example once every 12 or 24 months), and has my benefit already been used?
● Do I need to send my exam receipt along with the glasses receipt?
● What is the deadline for submitting a claim after I buy?
● Where do I send it, and is there an online option?
● What does the receipt have to show?
Write down the date, the representative's name, and what they said.
One more check: some plans sold as “eye care insurance” are discount plans. You pay a monthly fee for reduced prices, but they don't reimburse purchases. Your insurer can tell you which kind you have.
|
Before you buy: compare the real cost Take Aldermiller's price and subtract what your plan would reimburse. That is your real cost. Compare it with the in-network price for a like-for-like pair: same lens type, material, and coatings. Look at when your benefit resets. Many plans reset every 12 or 24 months, and unused benefits usually don't roll over. Remember that lens options such as coatings and special lenses may be reimbursed differently from frames. |
Step 2: Order and Pay in Full
Out-of-network claims are for purchases you have already paid for. Insurers' own claim forms ask for itemized, paid receipts, and one says outright that services must be paid in full to receive benefits [1]. Use the card or payment method you want to claim against. If you plan to use an FSA or HSA, see the section below.
Step 3: Get Your Itemized Receipt From Aldermiller
After you order, we email you a receipt. You can find it in your order confirmation email. If you need another copy, or a more detailed one for your insurer, email hello@aldermiller.com with your order number. Most insurers want a receipt that lists each item and its price, shows that you paid, and gives the date of purchase and the seller's name and contact details. Several forms also ask for the lens type, such as single vision or progressive [2].
Please note that Aldermiller does not provide eye exams, so we can't give you an exam receipt. Send your eye doctor's receipt separately if your insurer asks for it.
Step 4: Fill In Your Insurer's Claim Form
Most insurers have an out-of-network claim form, on their website or in your member account. A few have an online claim option. Complete every section, including your member ID, the patient's name and date of birth, the date of purchase, and the price of each item. Costs on the form should match the receipt, and most forms ask you to sign and date it.
Some forms ask for a provider's name, address, and phone number, and a few have signature lines for a provider. For those, enter Aldermiller's information. If your insurer insists on a provider's signature or an in-network provider's credentials, email us and we'll do what we can, though we may not be able to sign forms meant for in-network providers.
Step 5: Send It, and Keep Copies
● Attach the itemized receipt. Send every receipt for the claim together, because one insurer's form says they must be submitted at the same time [3].
● Use the submission method on the current form: online, mail, or fax.
● Keep a copy of everything, plus proof of mailing if you send it by post.
● Submit promptly. One insurer's form gives one year from the date of purchase [4], and others set different deadlines, so ask yours.
Step 6: Follow Up
Processing times differ. One insurer's form asks you to allow at least 14 days, with payment mailed within seven days after processing [1]. If you haven't heard back after several weeks, call the number on your ID card with your claim details. We aren't part of the claim, so we can't see its status, but we can send another copy of your receipt if you need it.
Where to Find Each Insurer's Claim Information
Addresses and phone numbers change, and employer plans often use different ones. Always use the contact details on your member ID card and on the current claim form for your plan. The table lists what each company's own claim documents showed when we checked on November 1, 2026.

Tips to Avoid a Denial
● Check your benefit period first. If you already used your frame or lens benefit this year, the claim may be denied.
● Match the form to the receipt. Prices and dates should agree.
● Don't leave out items. Include frames, lenses, and each coating or upgrade as listed on the receipt.
● Send everything at once. Partial claims can be rejected as duplicates.
● Watch for sales tax rules. Some forms ask for amounts without sales tax.
● Ask about discounts. Some insurers reduce reimbursement for discounted or promotional purchases.
● Don't wait. Deadlines can be as short as a few months.
Using an FSA or HSA Instead
A flexible spending account (FSA) or health savings account (HSA) is separate from vision insurance. The IRS says you can include amounts you pay for eyeglasses needed for medical reasons as medical expenses [11]. Your account is separate from vision insurance, and you don't need vision insurance to use it. If your FSA or HSA card works at checkout, you may not need a claim form. If your administrator asks for documentation, send them the itemized receipt from your order, or email us for another copy. Rules differ by plan, so check what your administrator accepts. Prescription eyeglasses are the clearest case. Non-prescription glasses, reading glasses, and sunglasses may need a doctor's note or may not qualify, so ask your administrator first. FSA money often has to be spent by the end of the plan year, so check your deadline before you shop.
If your card is declined at checkout, you can pay another way and email us for an invoice to send to your administrator.
Frequently Asked Questions
Why doesn't Aldermiller bill my insurance?
We are an independent online shop and are not part of any insurance network. That keeps our pricing simple, and it means you submit claims yourself.
How much will I get back?
Only your insurer can tell you. Out-of-network amounts are usually lower than in-network benefits, and some plans pay nothing for out-of-network eyewear.
Can you fill out the insurance form for me?
We can provide an itemized receipt and the business information the form asks for. We can't complete the member sections, and we may not be able to sign forms meant for in-network providers.
Can I use my insurance and an FSA or HSA together?
Sometimes. You may be able to use your FSA or HSA for what the insurance doesn't pay, but you can't be reimbursed twice for the same amount. Ask both administrators.
Will you tell me if my plan will pay?
We can't check benefits for you. Your insurer or your plan documents are the source for what they will pay.
